Too often, we design our own demise!
Surely, you can instantly think of that intersection through which you pass daily and dread every time you approach it because you know that it was engineered to fail pretty much 24/7.
Alas, you can think of several ways to fix the broken design, but you’re not a degreed civil engineer, so the authorities aren’t likely to listen to you.
It could be a left turn lane that’s impossibly short such that the 3rd car in line is sticking its butt into the travel lane thereby back up traffic EVERY time you are in a hurry.
Perhaps it’s one of those brilliant intersections at which 4 or 5 roads converge in a 100 yard span of pavement . . . Common sense might call for a rotary . . . or if only that drug store were to disappear, the intersection would suddenly become exponentially easier to navigate.
How ’bout that business we visit regularly that just doesn’t “click?” If we were all credentialed Organizational Development specialists, we could fix their issues with a few simple tweaks, but why would THEY listen to US because we aren’t credentialed.
Sadly, the design often creates the demise. Even the best intentioned, warm hearted people often have little traction with their design input.

It could be communication foibles or process headaches.
Whatever it is, my surmise is that if you were to do nothing more than simplify your ways and processes continually, this would be a darn good start towards dissolving the gridlock.
